Download Forms for your tubal ligation reversal
Before we can begin your tubal
ligation reversal we will need to obtain a copy of your Operative Note
and Health History. We will be happy to mail you a copy of the Operative
Note Request, the Health History form and send you a brochure if you will
email us your address.
Simply call our office at 1-877-588-5594 for more information. Also, for
your convenience, we have made these forms available online for you to download
and fax to our office. If you have any questions or have problems downloading
the forms please feel free to give us a call.
We do this day in and day out. Some health care facilities can be a pain
about providing records even though it is a requirement for accreditation
and, in some states, required by law. Over the years we have learned a lot
about how to get through the red tape.
Doctors don’t often have the op notes and sometimes we try them when
the hospital has destroyed the op note but we try the hospital first. Some
of our patients don’t want their doctors to know that they are considering
tubal reversal for various reasons. This is another reason we try the hospital
first.
It is not the end of the world if we cannot get your op note. Sometimes
we can talk to our tubal reversal patients and learn enough so we don’t
need the op note. When these things don’t work, we have a “look
and go” option. We make a small incision and see if you are reversible.
If things look okay, we go ahead and do your reversal if not we wake you
up and send you home with half your money refunded.
